Cacti has been a staple in network monitoring since 2002. It polls SNMP-enabled devices at configurable intervals and stores the data in round-robin databases, then renders that data into customizable graphs. You get a web-based interface where you can add devices, define data sources, and build graph templates without writing scripts. It supports remote data collectors (spines), so it scales from a small home lab to large enterprise networks with thousands of hosts. Templates from the community cover common gear like Cisco switches, Linux servers, and even UPS units. The graphing engine gives you fine control over colors, line styles, and stacked areas. There is also alerting built in: you can set thresholds on any data source and get notified via email or syslog. The interface is a bit dated looking, but it is fast and the feature set is mature. The Cacti Group maintains the project, and there are paid plugins for things like weathermap-style network maps and advanced reporting. If you need a reliable, self-hosted SNMP graphing tool with a large user base, Cacti is a solid pick.
